  • Organ and glandular meats. Avoid meats such as liver, kidney and sweetbreads, which have high purine levels and contribute to high blood levels of uric acid.
  • Red meat. Limit serving sizes of beef, lamb and pork.
  • Seafood. Some types of seafood — such as anchovies, shellfish, sardines and tuna — are higher in purines than are other types. But the overall health benefits of eating fish may outweigh the risks for people with gout. Moderate portions of fish can be part of a gout diet.
  • High-purine vegetables. Studies have shown that vegetables high in purines, such as asparagus and spinach, don't increase the risk of gout or recurring gout attacks.
  • Alcohol. Beer and distilled liquors are associated with an increased risk of gout and recurring attacks. Moderate consumption of wine doesn't appear to increase the risk of gout attacks. Avoid alcohol during gout attacks, and limit alcohol, especially beer, between attacks.
  • Sugary foods and beverages. Limit or avoid sugar-sweetened foods such as sweetened cereals, bakery goods and candies. Limit consumption of naturally sweet fruit juices.
  • Vitamin C. Vitamin C may help lower uric acid levels. Talk to your doctor about whether a 500-milligram vitamin C supplement fits into your diet and medication plan.
  • Coffee. Some research suggests that drinking coffee in moderation, especially regular caffeinated coffee, may be associated with a reduced risk of gout. Drinking coffee may not be appropriate if you have other medical conditions. Talk to your doctor about how much coffee is right for you.
  • Cherries. There is some evidence that eating cherries is associated with a reduced risk of gout attacks.

“I caught on to the whole Suboxone thing,” said Kyle Berry, a recovering addict from Grove City who used to obtain prescriptions for the drug.

Suboxone also is a popular street drug, one that state reports on drug-abuse trends say is “highly available for illicit use.” Berry saw that a prescription could be a means to other ends.

Drug dealers happily front the money for so-called “cash-n-carry” clinic visits in exchange for a share of the Suboxone, he said. Or, addicts sell it themselves, typically $15 to $20 for one of the “film strips” or pills.

Berry said he wasn’t cut off when he failed the urine tests that prescribers are supposed to use to check for the presence of buprenorphine and other drugs. No one called him in for a pill count, and he wasn’t questioned about the proof-of-counseling documents that he forged multiple times.

“The doctors poorly regulate it,” he said. “And if I bring somebody else in, that’s 25 bucks off.”


Used properly as part of a comprehensive recovery program, Suboxone can help drug users get off of heroin or opioid painkillers without the excruciating effects of withdrawal. Those who sell or buy it on the street, however, might take it only sporadically, to supplement or substitute for heroin and stave off being “dope sick.”

The number of doctors who each prescribe millions of dollars of medications annually in Medicare’s drug program has soared, driven by expensive hepatitis C treatments and rising drug prices overall, federal data obtained by ProPublica shows.

The number of providers who topped the $5 million mark for prescriptions increased more than tenfold, from 41 in 2011 to 514 in 2015. The number of prescribers—mostly physicians but also nurse practitioners–exceeding $10 million in drug costs jumped from two to 70 over the same time period, according to the data.

Most of the doctors atop the spending list prescribed Harvoni or Sovaldi, relatively new drugs that cure hepatitis C. Other providers on the list prescribed pricey drugs to treat cancer, multiple sclerosis and rheumatoid arthritis.

Medicare’s drug program, known as Part D, covers more than 41 million seniors and disabled people. In 2015, it accounted for $137.4 billion in drug spending, before factoring in rebates from drug companies. That was up from $121.5 billion a year earlier.

“The trends in this space are troubling and don’t show any signs of abating,” said Tim Gronniger, deputy chief of staff at the Centers for Medicare and Medicaid Services, the federal agency that runs Medicare. “It’s going to be a pressure point for patients and the program for the foreseeable future.”

During the recent presidential campaign, both Hillary Clinton and Donald Trump pledged to tackle the rising costs of prescription drugs. Since his election, however, President-elect Trump’s transition agenda for health care hasn’t featured the topic, a shift the Los Angeles Times reported.

Medicare has released top-level data on drug spending for 2015, including the number of doctors who prescribed medications worth more than $1 million. But the agency has only published data on individual doctors up to 2014.

Dr. Ben Thrower, medical director of the Multiple Sclerosis Institute at the Shepherd Center in Atlanta, was near the top of the list in 2014. He prescribed medications costing $11.5 million that year, mostly for multiple sclerosis drugs. “We get that it’s very expensive,” Thrower said. “I think all the MS providers working in the U.S. would like to see the costs go down.” But prices have climbed steadily in recent years for drugs used to treat the neurological condition, even those that have been on the market for quite a while.

Most of the spending on Thrower’s prescriptions – $8.5 million — was for MS drugs Tecfidera and Copaxone, which can slow progression of MS and reduce the chance of relapse. Thrower has received payments from the makers of those and other MS drugs, but said he cut ties with the companies in January of this year.

“It was kind of exciting when the first one came out,” Thrower said. “The problem we’ve seen is the cost for these drugs has just gone up and up and up.”

Thrower said he no longer prescribes Tecfidera to new patients because it can lower white blood cell counts, putting them at risk for infections.

Just because a doctor prescribes costly drugs doesn’t mean he or she has done anything wrong, Gronniger said. “It’s much more about drug pricing … than it is about the behavior of any individual physician, many of whom are equally concerned about the price of these products as we are.”

Today, ProPublica is updating its Prescriber Checkup online tool, which allows you to look up your doctor and see how his or her prescribing in Medicare Part D compares to others in the same specialty and state. Our tool covers the year 2014. You can compare the percentage of each doctor’s prescriptions that were for brand-name medications, the average cost per prescription and the average number of prescriptions per patient, among other things.

Allyson Funk, a spokeswoman for the Pharmaceutical Research and Manufacturers of America, the industry trade group, said Medicare’s figures leave out important context.

“It is important to note physicians’ prescribing patterns are dynamic and based on individual patient needs,” she said in a statement. “When looking at Medicare Part D, government data on spending at the point of sale does not include the substantial rebates for brand name medicines negotiated between manufacturers and plans and therefore does not accurately reflect actual prescribing dollars or program spending.”

These rebates are confidential by law, but Medicare said this week that the average rebate for brand-name drugs in 2014 was 17.5 percent.

The new data on high-spending prescribers is the latest indication of the burden of drug prices on government health programs.

In the past couple of years, Medicare’s drug tab has surged, in large part because it picks up the vast majority of the cost of drugs once enrollees exceed a certain threshold each year. In 2015, beneficiaries over the limit, which was $4,700, spent $51.3 billion on drugs. In 2013, the figure was $27.7 billion, government data shows. (Taxpayers, through Medicare, pick up 80 percent of the cost of this so-called catastrophic benefit.) The Associated Press first reported the ballooning cost in July.

Another sign could be seen in a drug dashboard released by Medicare this week. It showed that 1 percent of drugs prescribed in the Part D program accounted for more than one-third of the program’s cost in 2015 (before rebates). Some drugs were incredibly expensive. The drug H.P. Acthar Gel, used to treat several conditions, such as multiple sclerosis relapses and a rare kidney disease, cost an average of $162,371 for each of its 3,104 users—a higher per-user cost than any other drug in the program. That’s up significantly from several years ago, when ProPublica flagged the drug’s expense.

Harvoni, used by more than 75,000 people, cost an average of $92,847 per person, for a total cost of $7 billion (also before any rebates).

Michael Chernew, a professor of health care policy and director of the Healthcare Markets and Regulation Lab at Harvard Medical School, said the number of high-dollar prescribers in Medicare doesn’t surprise him. Given the increased cost of prescription drugs and the latest treatment advances, “the entire distribution is shifting to the right.”

Chernew said that, in the long term, Medicare and insurance companies have to examine the total cost of caring for patients with certain diseases, taking into account drugs, hospital visits, medical tests and more. Only then can anyone tell if certain expenses, such as a pricey new drug, are justified.

For now, he said, “How do we know what’s good or bad?”

Physicians say their top priority must be the patients in front of them, not the costs to the system.

Dr. Bruce Bacon, a liver specialist at St. Louis University, had the highest total Part D drug costs in 2014, $22.7 million. He was a frequent prescriber of Sovaldi and Olysio, another expensive hepatitis C medication.

Bacon did not return a call for comment for this story. In a 2015 interview, he said he did not realize his prescriptions were so costly to Medicare.

“I really don’t think about the cost,” he said. “I think about taking care of the patients. Should I not take care of the patients because the cost is expensive?”

Thrower, the multiple sclerosis specialist, said the high cost of drugs frustrates him and his colleagues, but ultimately the successful treatment of patients comes first.

“On one hand, we get that,” he said. “On the other hand, when you’re sitting in the exam room and looking someone in the eye, you can’t say, ‘I’m not going to treat you because of the cost.’”

“The only way to get them off drugs is to let them OD,” opioid epidemic

The story of the opioid epidemic begins with good intentions. In the 1980s, many prominent doctors began to argue that the medical community was doing too little to treat chronic pain, and that the psychological and emotional impact of that suffering was causing damage far beyond the identifiable injuries. Physicians were encouraged to believe their patients when they complained about pain, and to take aggressive steps to alleviate those symptoms.
At the same time, the Reagan administration was removing regulations that barred pharmaceutical companies from advertising directly to consumers. Massive companies like Purdue, Johnson & Johnson, and Mylan began to invest heavily in the production of pain pills. They also found ways to frame powerful opioids as a safe treatment for chronic pain. Purdue, in pushing oxycontin, told doctors, patients, and regulators that the medicine posed a lower risk of addiction and abuse than many of the common options already on the market.
The result was a flood of highly potent pain pills into the American bloodstream. Between 1999 and 2014, sales of opioid painkillers increased more than four fold. Not surprisingly, the rate of overdose deaths also quadrupled. Between 2007 and 2012, over 780 million doses of oxycodone and hydrocodone were shipped to West Virginia alone. That’s 433 pills for every person in the state — higher if you don’t count children. The drug companies reaped tens of billions in profit.

Sometimes, strong medications called opioids are needed to control pain. Opioid pain relievers are generally safe when taken for a short time as prescribed by your doctor, but they can become addictive, especially if they are misused. Regular use can lead to dependence. Never take opioids in greater amounts or more often than prescribed.
Using opioids can also increase risk for fallsdizziness, and other ailments in older adults.
Becoming addicted to prescription pain medicine can happen to anyone, including older adults. Sometimes, these treatments are the only ones available that can help. But, sometimes other treatments can and should be tried first or can be used intermittently or simultaneously. So, ask your doctor if there is another medicine or a non-medicine alternative you can try. Tell your doctor if you or a family member has a history of alcohol or drug abuse.

An Indiana doctor accused of operating a “pill mill” saw as many as 100 patients in a single day and traded pain pills for work on his farm, federal investigators say.

Dr. Tristan Stonger faces 55 felony charges stemming from a three-year investigation by the Drug Enforcement Agency, which spent months surveilling his Pain Management Centers of Indiana office in Peru, The Kokomo Tribune reported. He also operated offices in Bloomington and Indianapolis.

The charges Stonger faces include dealing in a controlled substance, possession of a narcotic drug, corrupt business influence and insurance and Medicaid fraud.

Greg Westfall, a DEA assistant special agent in charge, said Stonger faces “serious charges, considering the crisis we have throughout the U.S. with our epidemic of opiate-based drugs.”

“We charge those who exploit the weakness of addiction and bring those people to justice,” Westfall said.

The investigation revealed the Peru clinic was open only on Fridays, when more than 100 patients would sometimes visit the office, a probable cause affidavit said. Investigators determined Stonger spent only two to five minutes with each patient and did not perform any physical exams before writing them prescriptions, the affidavit said.

At least two patients told investigators they were employed by Stronger at his farm near Bunker Hill, where they worked on his vehicles or fed animals in exchange for pills.

On one occasion, one of the workers reported he was baling hay when Stonger said “I’ll bet you a bottle of hydro that you can’t get that bale up there.” The worker told investigators he then loaded the hay and Stonger gave him a bottle of the prescription opiate painkiller hydrocodone that he had at his house.

Another worker said Stonger wrote him a prescription while he was at the farm even though Stonger knew he was addicted to pills.

Agents identified two patients who died from overdoses after Stonger refilled their prescriptions, the newspaper reported.

On Jan. 27, Stonger was being held in the Miami County Jail on $300,000 bond.

Stonger also is awaiting trial after initially being arrested in April in Marion County on charges he traded prescription pills for sex with one of his female patients. His physician’s license to practice medicine in Indiana was suspended days later.

Easy To Use:

Unfold The Tinkle Belle soft liner outward so that the spout is fully extended.
Flex the hard-shell portion outward if desired to utilize a more open trough area. You can repeat as needed-The Tinkle Belle is flexible and durable!
Move panties or clothing to the side or down.
Stand with feet shoulder width apart.
Slide The Tinkle Belle into your clothing or fly.
Place the hard-shell portion of The Tinkle Belle next to your body under your pelvic area. It should be placed directly under your urethra so that urine flows directly into the hard-shell portion of the accessory. Most women find that the back edge of the Tinkle Belle is positioned under the center of the vaginal opening. Do not insert.
Utilize the thumb rests to point the spout DOWN for proper seal. This will allow The Tinkle Belle to seal against your body. Make sure any clothing/fly/zipper is under the hard-shell portion of The Tinkle Belle. The thumb rests should be accessible in front of clothing.
When properly positioned, the end of the spout should be the lowest point pointed downward.
Push your rear backwards to let gravity do its job. (yes, looking down does help!)
Relax and #letpeedomring.
When finished, keep The Tinkle Belle pointed down.
To wipe, slowly pull the Tinkle Belle forward with gentle pressure utilizing the extreme comfort lip. This will catch any last drops and leave you feeling clean and dry.
Shake any remaining drops off of your Tinkle Belle Accessory.
To store your Tinkle Belle on the go, fold back the soft spout under the hard-shell and keep it in the supplied carry case. For long term storage it is recommended that you store your Tinkle Belle with the spout deployed straight to avoid fold memory. The spout can also be stored by folding it to the side, but it may maintain "fold memory." You can fix this by pinching it to the opposite side a few times to straighten the spout.

Question:Is this supposed to fit over the outer lips, or in between the inner and outer lips? it seems pretty narrow to fit over both. Thanks!

Question:Is this made of soft rubber type material? I have had three different pee styles crack down the middle so they were unusable. Would this crack?
Answer:No, it does not crack regardless of how you bend it. You can sit on it, drive over it, or throw it off a cliff! Lol. It is a rigid material that won’t collapse, with a spout of a more flexible material.

I bought this product to use while traveling as I occasionally find myself in countries with less than hygienic restroom facilities. For anyone who has used the Pstyle, this product is very similar to use. It's easy to place and requires minimal exposure. I've been able to use it with all different clothing options, including jeans.

The main difference with this product is the addition of a softer material to the front half of the product that allows it to be folded in half for storage and carrying, which appealed to me as a traveler. This softer material also lines the inside and edge of the hard plastic portion making it easier to grip and also more comfortable to "wipe" with as the hard plastic doesn't touch your skin the way it does with the Pstyle. I also like the thumb rests which make it easier to push down and achieve the proper angle for use with thicker clothing like jeans or leggings.

For me this product is the best of both worlds. It's easy to use with minimal practice like the Pstyle, but with the added bonus of increased portability and comfort.

The carrying case that comes with it is nice too. It's reasonably discreet with only a small tag showing the product name and the inside is easily wiped clean.

I ordered 2 of them. I do have a go girl and a Venus to Mars. I do really like the Venus to Mars and the tinkle belle setup a lot better then the go girl. I really do like how the tinkle belle is longer, it folds up and comes with pouches.

This is the BEST female urinal device I have EVER purchased and have tried nearly 15 others.  As a women BSA leader, mom and avid kayaker and camper the Tinker Belle works flawlessly while standing, squatting and even sitting. The best part is I am able to slide the Tinker Belle through my open zipper, align it up to my body and go (now I know why men always pee on trees).  I have also been able to use it while sitting in my Hobie Kayak all I do is the same as above but insert the spout into my collapsible urinal and go. All in all I am overly impressed at the ease of use, functionality and ergonomic design. The instructions are incredibly well written and easy to follow. The compact design allows for easy travel. I had been very happy with my Freshette urinary device, but discovered the Tinker Belle through an Instagram ad. After laughing at their hilarious videos, of course I bought one. I like how flexible the TB is, especially compared to the Freshette, and that it folds down and fits into a provided carrying case. I use this mostly at festivals and burns, where port-o-potties are something to be (rightly) feared. The Tinker Belle would be perfect for hiking as well.
Be warned though - if you've been holding it for a while and you've got a lot of urine to relieve, don't release the food gates with full force!!! The reservoir isn't very deep (the Freshette wins out here), and you'll have a bad, wet time of it. Learn from my embarrassing mistake.
Practice makes perfect.

For any females that appreciate being outdoors exploring nature, we all know the potential discomfort involved with having to drop your drawers in the middle of the woods, baring all, just so you can pee. It's embarrassing if someone follows down the trail behind you, and can be downright uncomfortable on your bare bottom if it's cold/windy/rainy/etc. Enter the Tinkle Belle! I own this one and another funnel, and this one is definitely better. The other funnel requires a little practice to make sure you orient it correctly to make sure the liquid goes the right way. Suboxone

Suboxone contains naloxone, an opioid antagonist, as well as buprenorphine. As a prescription, Suboxone is administered sublingually (under the tongue). Buprenorphine has good bioavailability when taken sublingually, but poor availability when injected. Researchers at the University of Nottingham (UK) have found that these tumour-associated antigens (TAAs) are good indicators of cancer, and now they have developed panels of TAAs that are known already to be associated with breast cancer to detect whether or not there are autoantibodies against them in blood samples taken from patients.


In a pilot study the researchers, who are part of the Centre of Excellence for Autoimmunity in Cancer (CEAC) group at the School of Medicine, University of Nottingham, took blood samples from 90 breast cancer patients at the time they were diagnosed with breast cancer and matched them with samples taken from 90 patients without breast cancer (the control group).

They used screening technology (protein microarray) that allowed them to screen the blood samples rapidly for the presence of autoantibodies against 40 TAAs associated with breast cancer, and also 27 TAAs that were not known to be linked with the disease.

Presenting the research at the NCRI Conference, Ms Daniyah Alfattani, a PhD student in the group, said: "We were able to detect cancer with reasonable accuracy by identifying these autoantibodies in the blood."

The researchers identified three panels of TAAs against which to test for autoantibodies. The accuracy of the test improved in the panels that contained more TAAs. The panel of five TAAs correctly detected breast cancer in 29% of the samples from the cancer patients and correctly identified 84% of the control samples as being cancer-free. The panel of seven TAAs correctly identified cancer in 35% of cancer samples and no cancer in 79% of control samples. The panel of nine antigens correctly identified cancer in 37% of cancer samples and no cancer in 79% of the controls.

"We need to develop and further validate this test," said Ms Alfattani. "However, these results are encouraging and indicate that it's possible to detect a signal for early breast cancer. Once we have improved the accuracy of the test, then it opens the possibility of using a simple blood test to improve early detection of the disease."

The researchers are now testing samples from 800 patients against a panel of nine TAAs, and they expect the accuracy of the test to improve with these larger numbers.

"A blood test for early breast cancer detection would be cost effective, which would be of particular value in low and middle income countries. It would also be an easier screening method to implement compared to current methods, such as mammography," said Ms Alfattani. The researchers estimate that, with a fully-funded development programme, the test might become available in the clinic in about four to five years.

A similar test for lung cancer is currently being tested in a randomised controlled trial in Scotland, involving 12,000 people at high risk of developing lung cancer because they smoke. They have been randomised to have (or not) an autoantibody blood test called ELISA (Early CDT-Lung). Participants who test positive for the autoantibodies are then followed up with a CT scan every two years in order to detect lung cancer in its early stages when it is easier to treat.

The CEAC group is also working on similar tests for pancreatic, colorectal and liver cancers. Solid tumours like these, as well as lung and breast cancer, represent around 70% of all cancers. "A blood test capable of detecting any of these cancers at an early stage is the over-riding objective of our work," concluded Ms Alfattani.

The results from this pilot study for a blood test to detect early breast cancer are promising and build on this research group's expertise in other cancers, such as lung cancer. It's obviously early days but we look forward to seeing the results from the larger group of patients that are now being investigated."
