Who started calling Doctors as Providers? .can somebody give me good reasons to call physicians providers?

Who started calling Doctors as Providers? 

Insurance companies and hospital administrators with a vested monetary interest in cutting physician compensation.
Crooked politicians from both sides of the Aisle who want to fool the general public.
Physician Assistants, family nurse practitioners, Podiatrists and  Chiropractors who want to get the same title as Physicians without having to go through the same grind physicians go through.

Some specialists among the physicians who are in private practice and want to make more money. They employ midlevel providers like PAs and NPs to deal with some routine cases. So that under the guise of supervised specialist care, they can  charge  patients more
As a  Board Certified Internist with more than  35 years of practical experience, I expect a specialist physician to see my patient and send back a useful and logical opinion and plan of care. I do not want my patient to be seen by someone who learnt some cookbook protocols. if I wanted that I can do a better job by consulting   Google and  Uptodate.
