so much so the normal way of thinking and transcribing a free flow history is interrupted badly and the physician is straightjacketed into clicking and adding meaningless phrases . murmur present ,
skin lesion present. etc

patients bring a few hundred pages of printed notes from their hospital/ER visit stay and expect PCp to know everything which happened to them in the hospital.including what kind of colour the shit was on the 3rd day, 4th time.
Why are we physicians putting up with all this shit ?
