Teaching Points
- The treatment for lung abscesses is prolonged antibiotic therapy. The regimen should cover both anaerobes and aerobes.
- The duration of antibiotics is three to six weeks, and serial x-rays are required to follow the progression of the disease.
- If there is no response to antibiotics there can be a bronchial obstruction, cancer, resistant bacteria, or tuberculosis.
- Surgery is rare for a lung abscess. Percutaneous catheter drainage can be tried. The belief that one can create a bronchopleural fistula from a chest tube is not supported by the evidence.
