"Interdental" lisping is produced when the tip of the tongue protrudes between the front teeth and "dentalised" lisping is produced when the tip of the tongue just touches the front teeth.
The "lateral" lisp is where the [s] and [z] sounds are produced as a result of air-flow being pushed over the sides of the tongue. It is also called 'slushy ess' or a 'slushy lisp' due to the wet, spitty sound. This is fine when it comes to producing the "L" tones, but it sounds slushy or wet when trying to make out the "S" tone. The symbols for these lateralised sounds are in the Extended International Phonetic Alphabet for disordered speech ([ʪ] and [ʫ]).
A "nasal lisp" occurs when part or the entire air stream is directed through the nasal cavity.
A "trident lisp" results in a high-frequency whistle of hissing sound caused by stream passing between the tongue and the hard surface.
A "dentalized lisp" does not have a sibilant quality, resulting from occluding the breath stream in the oral cavity, usually by placing the tongue too far forward of not allowing for a narrow, groved passageway for the breath stream to pass between the tongue and alveolar ridge.

Lisps may have either physical or psychological causes. Most lisps are caused by errors in tongue placement within the mouth. The most frequently discussed of these problems is tongue thrust in which the tongue protrudes beyond the anterior incisors. This protrusion affects speech as well as swallowing and can lead to lisping. Ankyloglossia or tongue-tie can also be responsible for lisps in children. However it is unclear whether these deficiencies are caused by the tongue-tie itself or the muscle weakness following the correction of the tongue-tie.[2] Overbites and underbites may also contribute to lisping. Temporary lisps can be caused by dental work, dental appliances such as dentures or retainers or by swollen or bruised tongues.
Treatment Treating lisps in children usually involves speech therapy treatments and is generally successful. Speech therapy sessions include a wide variety of activities and speech drills, though what specifically happens in any given session will depend upon many variables. The content of these sessions will be affected by the length of each therapy session (usually between a half hour and one hour), its location (whether at home, school or a private facility), the age of the child involved, whether the therapy session is private or involves a group, and the type of lisp that is being treated.
One popular method of correcting articulation or lisp disorders is to isolate sounds and work on correcting the sound in isolation. The basic sound, or phoneme, is selected as a target for treatment. Typically the position of the sound within a word is considered and targeted. The sound appears in the beginning of the word, middle, or end of the word (initial, medial, or final).
Take for example, correction of an "S" sound (lisp). Most likely, a speech-language pathologist (SLP) would employ exercises to work on "Sssssss". Starting practice words would most likely consist of "S-initial" words such as "say, sun, soap, sip, sick, said, sail". According to this protocol, the SLP slowly increases the complexity of tasks (context of pronunciations) as the production of the sound improves. Examples of increased complexity could include saying words in phrases and sentences, saying longer multi-syllabic words, or increasing the tempo of pronunciation.
Using this method, the SLP achieves success with his/her student by targeting a sound in a phonetically consistent manner. Phonetic consistency means that a target sound is isolated at the smallest possible level (phoneme, phone, or allophone) and that the context of production must be consistent. Consistency is critical, because factors such as the position within the word, grouping with other sounds (vowels or consonants), and the complexity all may affect production.
Another popular method for treating a lisp is using specially designed devices that go in the mouth to provide a tactile cue of exactly where the tongue should be positioned when saying the "S" sound. This tactile feedback has been shown to correct lisp errors twice as fast as traditional therapy.
Using either or both methods, the repetition of consistent contexts allows the student to align all the necessary processes required to properly produce language; language skills (ability to formulate correct sounds in the brain: What sounds do I need to make?), motor planning (voicing and jaw and tongue movements: How do I produce the sound?), and auditory processing (receptive feedback: Was the sound produced correctly? Do I need to correct?). A student with an articulation or lisp disorder has a deficiency in one or more of these areas. To correct the deficiency, adjustments have to be made in one or more of these processes. The process to correct it is more often than not, trial and error. With so many factors, however, isolating the variables (the sound) is imperative to getting to the end result faster.
A phonetically consistent treatment strategy means practicing the same thing over and over. What is practiced is consistent and does not change. The words might change, but the phoneme and its positioning is the same (say, sip, sill, soap, …). Thus, successful correction of the disorder is found in manipulating or changing the other factors involved with speech production (tongue positioning, cerebral processing, etc.). Once a successful result (speech) is achieved, then consistent practice becomes essential to reinforcing correct productions.
When the difficult sound is mastered, the child will then learn to say the sound in syllables, then words, then phrases and then sentences. When a child is able to speak a whole sentence without lisping, attention is then focused on making correct sounds throughout natural conversation. Towards the end of the course of therapy, the child will be taught how to monitor his or her own speech, and how to correct as necessary. Speech therapy can sometimes fix the problem, but however in some cases speech therapy fails to work.
Permanent lisps can often be corrected through extensive oral operations. Often, when a patient has extreme overbite, causing a lisp, having orthodontic braces and rubber bands for an extended period of time will correct the issue, and resolve the lisp.

HISTORICAL PERSPECTIVE!
There is a long tradition of interest in communication disorder. From
Demosthenes with his pebbles, from Leonardo da Vinci with his drawings
and models of the vocal tract, from Pedro Ponce de Leon with his early
version of ' Total communication' in sixteenth-century Spain, and from the
divergence of opinion of L'Abbe de L'Eppee and Heineke in the
eighteenth century, students of speech pathology know that the inability of
some to use communication normally has always fascinated. However, this
historical review will be restricted to a very brief outline of changes in
emphases of assessment over the twentieth century.
In the early part of this century, speech and language therapists fell
broadly into two groups. The first was concerned with the mechanics of
speech and embraced the new knowledge derived from phonetics (e.g.
Aiken, 1910). Aiken's preface to the second edition of his book The Voice
states that ' it has become necessary to remodel and rewrite the whole work
and to regard it as an introduction to the practical science of phonology'
(p. I). The second group concentrated more on the psychological behaviour
of 'speech-disordered' people. Researchers in speech pathology,
notably in Berlin following Gutzman, in Vienna following Froeschels, in
London and in the United States, notably at the University of Columbia,
following Scripture, were fascinated by four problems: those of stuttering,
of voice, of 'dyslalia' and of dysphasia, almost to the exclusion of any
awareness of other disorders. Similarly the remedial practitioners, who in
the early part of the century came from a range of backgrounds, became
remedial specialists for specific client groups.

Had  a girl with  stammering problem come to USA from Hyderabad India  to undergo  speech and language  pathology treatment. one  way this  is being  corrected is  by  a device  placed in  her  one  ear which delays and also changes the tone and frequency of her own speech as feed back somehow this  correct the  stammering.
I do not have  any objective test results available but   My brother tells me  it  is working  quite well.
this  is  being  done by a  speech therapist in College station Texas.
One of theses days I will ask My  Bother.

"One of the earliest attempts to establish guidelines as to the extent of the
language-disordered population in childhood was undel1aken in Newcastle
in the 19405 and 19505 by Muriel Morley (1965). Her study was part of a
larger investigation into child development and disease following the
development of 1,142 children bom in Mayor June 1947 in Newcastle. In
Morley's study, children's speech was investigated at 3 years 6 months, 4
years 9 months, 6 years 6 months and 9 years 6 months. Morley categorized
children into four groups:
1) Those with delayed development of speech and language
2 )those with 'resolving difficulties of al1iculation'
3) those with 'persistent defects of articulation' and
4) stammerers.
In common with contemporary thinking, Morley's study used a diseasebased
medical model and introduced a taxonomy to childhood disorders of
speech and language. Of epidemiological interest is Morley's finding that at 6 years 6 months. 3.7 per cent of The children in her study had persisting difficulties with speech.(emphasis mine )

Poor adherence to appropriate medication therapy has been shown to result in complications, death, and increased health care costs.1-4 Medication adherence in patients with chronic conditions such as diabetes, hypertension, hyperlipidemia, asthma, and depression is a significant problem requiring intervention.5-12 According to the World Health Organization’s (WHO) World Health Report 2003, the degree of medication nonadherence is so great and the consequences are of such concern that more people worldwide would benefit from efforts to improve medication adherence than from the development of new medical treatments.13 In fact, a number of studies have shown that, in developed countries, patients with chronic conditions have adherence rates of 50% to 60%, despite evidence that medication improves quality of life and prevents death.5-10 In developing countries, when poor access to health care, possible lack of diagnosis, and limited availability of medications are taken into account, poor adherence threatens all efforts to treat chronic conditions such as diabetes, depression, and HIV/AIDS.11
The rate of patients who persistently take their medication for a chronic condition drops most significantly after the first 6 months of therapy.14-17 Acceptable rates of adherence in some trials are 80% or greater, while those for HIV are considerably higher (95%).17 Of all medication-related hospital admissions in the United States, 33% to 69% are related to poor medication adherence. The resulting costs are approximately $100 billion annually.17 It has been estimated that 125,000 deaths per year in the U.S. can be attributed to medication nonadherence.18 A retrospective analysis of patients aged under 65 years that was conducted between 1997 and 1999 concluded that diabetic patients with poor medication adherence had a 30% yearly risk of hospitalization, while those with high adherence (defined as following the medication regimen 80%-100% of the time) had a 13% risk.19 In the same analysis, total health care spending for patients with low medication adherence was almost twice that of patients with high adherence.
It was estimated in 2001 that more than 89,000 deaths from cardiovascular disease would have occurred in hypertensive patients aged 40 years and older who were not receiving antihypertensive medication therapy.20 Patients who are diabetic or who have heart disease are 48% less likely to die prematurely when they are adherent to their medication regimen.21 In addition, asthma patients who are adherent to their medications are 11% less likely to have an emergency room (ER) visit or to be hospitalized.21
When counseling about medication adherence for the three major disease states—diabetes, hypertension, and hyperlipidemia—it is important to educate the patient about the consequences of not appropriately treating these conditions, which can result in heart attack, heart failure, stroke, paralysis, and potentially permanent and devastating alterations to their quality of life. Pharmacists must paint a picture of the situation and remind patients why they are taking the medication in the first place.

Defining Adherence

It has been said that a primary determinant of treatment success is adherence to therapy.22 As defined by the WHO, adherence is the degree to which the patient’s behavior is in agreement with the health care provider’s recommendations. This is a collaborative approach between the patient and the provider in which the patient’s values, lifestyle, and beliefs coincide with the provider’s medical advice and opinion. Compliance, which is often confused with adherence, refers to a patient’s obedience to therapy under the authority of the provider, rather than to a collaborative approach. Another term that relates to adherence is persistence, which is the patient’s ability to continue taking the medication for the duration of treatment. When a medication is initiated, discussing the intended course of therapy with the patient has been shown to help keep the patient persistently following his or her medication regimen.12

Factors That Impact Adherence

The most consistently reported factors impacting adherence are low literacy, lack of health insurance coverage, poor social support, family instability, and homelessness.12 Forty-five percent of U.S. adults have a literacy level at or below eighth grade.12 It has been reported that one-half of adults lack the skills to complete tasks related to following medication-label directions, comprehending nutrition labels, describing symptoms, or using a map to locate health facilities.23 This can be a particular problem in ethnic groups that already have a low English-literacy level. Decreased health literacy has been reported with increasing age, such that adults aged 60 years may not understand basic materials such as medication labels. See TABLE 1 for tools used both to assess health literacy, medication literacy, and willingness to change behaviors and to evaluate the degree of social support available to the patient.24-27 Patient beliefs about the diagnosis and medication benefits also have been shown to affect the adherence rate regardless of whether the patient can afford the medication. This means that patients may have a low perceived need for the medication and more concerns about side effects, thus possibly choosing to forgo the cost of the medication based on their beliefs.28

As providers, pharmacists can use the patient education program Ask Me 3, which is designed to promote communication between health care providers and patients, to improve adherence and outcomes. The Ask Me 3 questions are: What is my main problem? What do I need to do? and Why is it important for me to do this?29 Pharmacists should instruct their patients to ask these three questions, and they should state them to the patient and answer them in the event that the patient does not ask. It is equally important that the patient rephrase what the pharmacist has communicated in order to verify the patient’s comprehension and to clarify whether the information fits the patient’s beliefs. When answering these questions, particularly with asymptomatic patients, it is helpful for pharmacists to remind patients of the consequences of failing to take their medication, such as a devastating or life-ending event (heart attack or stroke) in patients with cardiovascular disease.
Prerequisites for medication adherence may include being interested in one’s health and understanding the diagnosis; comprehending the impact or potential impact of the diagnosis; believing that what has been prescribed will help; and knowing precisely how to take the medication and for how long. Other prerequisites may be determining how to fit the medication into one’s daily routine; valuing the results of treatment more than the cost of therapy; believing that one can fulfill the treatment plan; and feeling that the provider has true concern for the patient as a person, as opposed to just treating the disease.12
The following steps have been shown to increase adherence: verifying the patient’s understanding of the disease and its treatment and providing education where gaps in understanding exist; correlating medication-taking with other daily routines; using medication organizers or charts; reducing the pill burden, if possible; providing care support (i.e., health care team); and recognizing any struggles or other sociobehavioral issues that may affect adherence.12

How to Overcome Adherence Challenges

Many different factors can affect adherence. The first step, and the one that is most often missed, is to identify which factors are preventing adherence. One way to identify adherence gaps is to perform a medication reconciliation—best done using pharmacy adjudication records—allowing for discovery of pharmacy-dispensing gaps. This might reveal the need for a simplification of the dosing regimen.30 See TABLE 2 for a list of factors that affect adherence and strategies for overcoming them.29,31,32

Strategies to overcome low health literacy include creating a shame-free environment that promotes open dialogue; using plain language versus medical terminology; providing written communication on a fifth-grade level; using picture or video instruction; involving family members in the instruction; having the patient repeat the instruction; and following up by telephone to confirm the patient’s understanding.12
The inability to pay for health care can be a deterrent to adequate adherence; however, the degree to which patients value their medication or perceive it to help their condition, along with their beliefs about side effects, can influence their decision to forgo treatment, regardless of cost.28 Patients with depression show a greater tendency to be deterred by their beliefs and medication costs, thereby putting those with diabetes and depression at even greater risk for medication nonadherence. This is true for patients who have depression with other comorbid conditions. Providers should initiate more in-depth discussions about medication cost concerns in patients with depression.28
Lack of prescription coverage can be a major roadblock to medication adherence. There are several options for patients who do not have prescription coverage, including Medicare Part D (for patients eligible for Medicare), Medicare Extra Help (for those who have Medicare but have limited income and resources), Medicaid programs, the Together Rx Access Card (25%-40% off brand-name prescription products), Partnership for Prescription Assistance (free medications to qualified patients), NeedyMeds Drug Discount Card (80% off the price of prescriptions), Coast2Coast Rx Card (free diabetes supplies and up to 55% off medication costs), and discount cards offered by some pharmacy benefit management companies and individual pharmaceutical companies.33-40 This list of discount cards is not all-inclusive. The $4 generic program is another option offered by some large chain pharmacies and supermarket pharmacies; in some cases, independent pharmacies offering this program may match pricing.41 All of these discount programs have restrictions, so check their respective Web sites (TABLE 3).33-41 Lastly, some employers offer reduced copayments or medication deductibles to patients who participate in a health program for chronic diseases.42

Conclusion

Poor adherence to medication regimens is multifactorial, involving the patient’s beliefs about and understanding of the diagnosis and how the medication will help his or her prognosis.22 Three important questions to answer for the patient are: What is the patient’s main problem? What does the patient need to do? and Why is it important for the patient to do this? After these questions have been answered, ascertain that the patient believes that this treatment will help. Afterward, probe for any issues that may result in low adherence, such as low literacy, inability to pay for the medication, and poor social support, among many others.29 The first step is to check medication adherence and persistence by way of a pharmacy medication reconciliation that utilizes pharmacy adjudication claims.12

Abstract

BACKGROUND:

People who are prescribed self-administered medications typically take less than half the prescribed doses. Efforts to assist patients with adherence to medications might improve the benefits and efficiency of health care, but also might increase its adverse effects.

OBJECTIVES:

To update a review summarising the results of randomised controlled trials (RCTs) of interventions to help patients follow prescriptions for medications for medical problems, focusing on trials that measured both adherence and clinical outcomes.

SEARCH STRATEGY:

Computerised searches to August 2001 in MEDLINE, CINAHL, The Cochrane Library, International Pharmaceutical Abstracts (IPA) PsychInfo, and Sociofile; bibliographies in articles on patient adherence; articles in the reviewers' personal collections; and contact with authors of original and review articles on the topic.

SELECTION CRITERIA:

Articles were selected if they reported an unconfounded RCT of an intervention to improve adherence with prescribed medications, measuring both medication adherence and treatment outcome, with at least 80% follow-up of each group studied and, for long-term treatments, at least six months follow-up for studies with positive initial findings.

DATA COLLECTION AND ANALYSIS:

Information on study design features, interventions and controls, and results were extracted by one reviewer and confirmed by at least one other reviewer. The studies were too disparate to warrant meta-analysis.

MAIN RESULTS:

For short-term treatments, one of three interventions reported in three RCTs showed an effect on both adherence and clinical outcome. Eighteen of 36 interventions for long-term treatments reported in 30 RCTs were associated with improvements in adherence, but only 16 interventions led to improvements in treatment outcomes. Almost all of the interventions that were effective for long-term care were complex, including combinations of more convenient care, information, reminders, self-monitoring, reinforcement, counselling, family therapy, and other forms of additional supervision or attention by a health care provider (physician, nurse, pharmacist or other). Even the most effective interventions did not lead to large improvements in adherence and treatment outcomes. Two studies showed that telling patients about adverse effects of treatment did not affect their adherence.

REVIEWER'S CONCLUSIONS:

The full benefits of medications cannot be realised at currently achievable levels of adherence. Current methods of improving adherence for chronic health problems are mostly complex and not very effective. Innovations to assist patients to follow medication prescriptions are needed.
