Stuttering (/ˈstʌtərɪŋ/; alalia syllabaris), also known as stammering (/ˈstæmərɪŋ/; alalia literalis or anarthria literalis), is a speech disorder in which the flow of speech is disrupted by involuntary repetitions and prolongations of sounds, syllables, words or phrases as well as involuntary silent pauses or blocks in which the person who stutters is unable to produce sounds.[1] The term stuttering is most commonly associated with involuntary sound repetition, but it also encompasses the abnormal hesitation or pausing before speech, referred to by people who stutter as blocks, and the prolongation of certain sounds, usually vowels and semivowels. For many people who stutter, repetition is the primary problem. Blocks and prolongations are learned mechanisms to mask repetition, as the fear of repetitive speaking in public is often the main cause of psychological unease. The term "stuttering" covers a wide range of severity, encompassing barely perceptible impediments that are largely cosmetic to severe symptoms that effectively prevent oral communication.
The impact of stuttering on a person's functioning and emotional state can be severe. This may include fears of having to enunciate specific vowels or consonants, fears of being caught stuttering in social situations, self-imposed isolation, anxiety, stress, shame, or a feeling of "loss of control" during speech. Stuttering is sometimes popularly associated with anxiety but there is actually no such correlation (though as mentioned social anxiety may actually develop in individuals as a result of their stuttering).[citation needed] Stuttering is not an indicator of reduced intelligence.[2]
Stuttering is generally not a problem with the physical production of speech sounds or putting thoughts into words. Acute nervousness and stress do not cause stuttering but they can trigger stuttering in people who have the speech disorder, and living with a highly stigmatized disability can result in anxiety and high allostatic stress load (i.e., chronic nervousness and stress) that reduce the amount of acute stress necessary to trigger stuttering in any given person who stutters, exacerbating the problem in the manner of a positive feedback system; the name 'Stuttered Speech Syndrome' has been proposed for this condition.[3][4] Neither acute nor chronic stress, however, itself creates any predisposition to stuttering.
The disorder is also variable, which means that in certain situations, such as talking on the telephone, the stuttering might be more severe or less, depending on the anxiety level connected with that activity. Although the exact etiology or cause of stuttering is unknown, both genetics and neurophysiology are thought to contribute. There are many treatments and speech therapy techniques available that may help increase fluency in some people who stutter to the point where an untrained ear cannot identify a problem; however, there is essentially no cure for the disorder at present.[5]

Developmental stuttering is stuttering that originates when a child is learning to speak and develops as the child matures into adulthood. Other disorders with symptoms resembling stuttering include Asperger's syndrome, cluttering, Parkinson's speech, essential tremor, palilalia, spasmodic dysphonia, selective mutism, and social anxiety.

Primary behaviors [edit]
Primary stuttering behaviors are the overt, observable signs of speech fluency breakdown, including repeating sounds, syllables, words or phrases, silent blocks and prolongation of sounds. These differ from the normal disfluencies found in all speakers in that stuttering disfluencies may last longer, occur more frequently, and are produced with more effort and strain.[6] Stuttering disfluencies also vary in quality: normal disfluencies tend to be a repetition of words, phrases or parts of phrases, while stuttering is characterized by prolongations, blocks and part-word repetitions.[7]
Repetition occurs when a unit of speech, such as a sound, syllable, word, or phrase is repeated and are typical in children who are beginning to stutter. For example, "to-to-to-tomorrow".
Prolongations are the unnatural lengthening of continuant sounds, for example,"mmmmmmmmmilk". Prolongations are also common in children beginning to stutter.
Blocks are inappropriate cessation of sound and air, often associated with freezing of the movement of the tongue, lips and/or vocal folds. Blocks often develop later, and can be associated with muscle tension and effort.[8]
Variability [edit]
The severity of a stutter is often not constant even for people who severely stutter. People who stutter commonly report dramatically increased fluency when talking in unison with another speaker, copying another's speech, whispering, singing, and acting or when talking to pets, young children, or themselves.[9] Other situations, such as public speaking and speaking on the telephone are often greatly feared by people who stutter, and increased stuttering is reported.[10]
Feelings and attitudes [edit]
Stuttering may have a significant negative cognitive and affective impact on the person who stutters. Joseph Sheehan, a prominent researcher in the field, has described stuttering in terms of the well-known analogy to an iceberg, with the immediately visible and audible symptoms of stuttering above the waterline and a broader set of symptoms such as negative emotions hidden below the surface.[11] Feelings of embarrassment, shame, frustration, fear, anger, and guilt are frequent in people who stutter,[12] and may actually increase tension and effort, leading to increased stuttering.[13] With time, continued exposure to difficult speaking experiences may crystallize into a negative self-concept and self-image. A person who stutters may project his or her attitudes onto others, believing that they think he or she is nervous or stupid. Such negative feelings and attitudes may need to be a major focus of a treatment program.[13]
Many people who stutter report about a high emotional cost, including jobs or promotions not received, as well as relationships broken or not pursued.[14]
Fluency and disfluency [edit]
Linguistic tasks can invoke speech disfluency. People who stutter may "move along a continuum from fluency to dysfluency."[15] Tasks that trigger disfluency usually require a controlled-language processing, which involves linguistic planning. In stuttering, it is seen that many individuals have fluency when it comes to tasks that allow for automatic processing without substantial planning. For example, singing "Happy Birthday" or other relatively common, repeated linguistic discourses could be fluid in people who stutter. Tasks like this reduce semantic, syntactic, and prosodic planning, whereas spontaneous, "controlled" speech or reading aloud requires thoughts to transform into linguistic material and thereafter syntax and prosody. Some researchers hypothesize that controlled-language activated circuitry consistently does not function properly in people who stutter, whereas people who do not stutter only sometimes display disfluent speech and abnormal circuitry.[15]
Sub-types [edit]

Developmental [edit]
Stuttering is typically a developmental disorder beginning in early childhood and continuing into adulthood in at least 20% of affected children.[16][17] The mean onset of stuttering is 30 months.[18] Although there is variability, early stuttering behaviours usually consist of word or syllable repetitions, and secondary behaviours such as tension, avoidance or escape behaviours are absent.[19] Most young children are unaware of the interruptions in their speech.[19] With early people who stutter, disfluency may be episodic, and periods of stuttering are followed by periods of relative fluency.[20]
Though the rate of early recovery is very high,[16] with time a young person who stutters may transition from easy, relaxed repetition to more tense and effortful stuttering, including blocks and prolongations.[19] Some propose that parental reaction may affect the development of chronic stutter. Recommendations to slow down, take a breath, say it again, etc. may increase the child’s anxiety and fear, leading to more difficulties with speaking and, in the "cycle of stuttering" to ever yet more fear, anxiety and expectation of stuttering.[21] With time secondary stuttering including escape behaviours such as eye blinking, lip movements, etc. may be used, as well as fear and avoidance of sounds, words, people, or speaking situations. Eventually, many become fully aware of their disorder and begin to identify themselves as "stutterers". With this may come deeper frustration, embarrassment and shame.[22] Other, rarer, patterns of stuttering development have been described, including sudden onset with the child being unable to speak, despite attempts to do so.[23] The child usually is unable to utter the first sound of a sentence, and shows high levels of awareness and frustration. Another variety also begins suddenly with frequent word and phrase repetition, and does not develop secondary stuttering behaviours.[23] Another way stuttering comes about is through child development. Many toddlers and preschool age children stutter as they are learning to talk, and although many parents worry about it, most of these children will outgrow the stuttering and will have normal speech as they get older. Since most of these children do not stutter as adults, this normal stage of speech development is usually referred to as pseudo-stuttering or as a normal dysfluency. As children learn to talk, they may repeat certain sounds, stumble on or mispronounce words, hesitate between words, substitute sounds for each other, and be unable to express some sounds. Children with a normal dysfluency usually have brief repetitions of certain sounds, syllables or short words; however, the stuttering usually comes and goes and is most noticeable when a child is excited, stressed or overly tired. Stuttering is also believed to be caused by neurophysiocology. Neurogenic stuttering is a type of fluency disorder in which a person has difficulty in producing speech in a normal, smooth fashion. Individuals with fluency disorders may have speech that sounds fragmented or halting, with frequent interruptions and difficulty producing words without effort or struggle. Neurogenic stuttering typically appears following some sort of injury or disease to the central nervous system. Injuries to the brain and spinal cord, including cortex, subcortex, cerebellar, and even the neural pathway regions.[citation needed]
Acquired stuttering [edit]
In rare cases, stuttering may be acquired in adulthood as the result of a neurological event such as a head injury, tumour, stroke, or drug use. The stuttering has different characteristics from its developmental equivalent: it tends to be limited to part-word or sound repetitions, and is associated with a relative lack of anxiety and secondary stuttering behaviors. Techniques such as altered auditory feedback (see below), which may promote fluency in people who stutter with the developmental condition, are not effective with the acquired type.[16][17][24]
Psychogenic stuttering may also arise after a traumatic experience such as a grief, the breakup of a relationship or as the psychological reaction to physical trauma. Its symptoms tend to be homogeneous: the stuttering is of sudden onset and associated with a significant event, it is constant and uninfluenced by different speaking situations, and there is little awareness or concern shown by the speaker.[25]
Causes of developmental stuttering [edit]

No single, exclusive cause of developmental stuttering is known. A variety of hypotheses and theories suggests multiple factors contributing to stuttering.[16] Among these is the strong evidence that stuttering has a genetic basis.[26] Children who have first-degree relatives who stutter are three times as likely to develop a stutter.[27] However, twin and adoption studies suggest that genetic factors interact with environmental factors for stuttering to occur,[28] and many people who stutter have no family history of the disorder.[29] There is evidence that stuttering is more common in children who also have concomitant speech, language, learning or motor difficulties.[30] Robert West, a pioneer of genetic studies in stuttering, has suggested that the presence of stuttering is connected to the fact that articulated speech is the last major acquisition in human evolution.[31] Another view is that a stutter is a complex tic.[32]
In a 2010 article, three genes were found to correlate with stuttering: GNPTAB, GNPTG, and NAGPA. Researchers estimated that alterations in these three genes were present in 9% of people who stutter who have a family history of stuttering.[33]
For some people who stutter, congenital factors may play a role. These may include physical trauma at or around birth, as well as cerebral palsy and mental retardation. For other people who stutter, there could be added impact due to stressful situations such as the birth of a sibling, moving, or a sudden growth in linguistic ability.[26][28]
There is clear empirical evidence for structural and functional differences in the brains of people who stutter. Research is complicated somewhat by the possibility that such differences could be the consequences of stuttering rather than a cause, but recent research on older children confirm structural differences thereby giving strength to the argument that at least some of the differences are not a consequence of stuttering.[34][35]
Auditory processing deficits have also been proposed as a cause of stuttering. Stuttering is less prevalent in deaf and hard-of-hearing individuals,[36] and stuttering may be improved when auditory feedback is altered, such as masking, delayed auditory feedback (DAF), or frequency altered feedback.[16][37] There is some evidence that the functional organization of the auditory cortex may be different in people who stutter.[16]
There is evidence of differences in linguistic processing between people who stutter and people who do not stutter.[38] Brain scans of adult people who stutter have found increased activation of the right hemisphere, which is associated with emotions, than in the left hemisphere, which is associated with speech. In addition reduced activation in the left auditory cortex has been observed.[16][28]
The capacities and demands model has been proposed to account for the heterogeneity of the disorder. In this approach, speech performance varies depending on the capacity that the individual has for producing fluent speech, and the demands placed upon the person by the speaking situation. Capacity for fluent speech, which may be affected by a predisposition to the disorder, auditory processing or motor speech deficits, and cognitive or affective issues. Demands may be increased by internal factors such as lack of confidence or self esteem or inadequate language skills or external factors such as peer pressure, time pressure, stressful speaking situations, insistence on perfect speech, and the like. In stuttering, the severity of the disorder is seen as likely to increase when demands placed on the person's speech and language system exceed their capacity to deal with these pressures.[39]

Neuroimaging of developmental stuttering in adults [edit]

Several neuroimaging studies have emerged in order to identify areas associated with stuttering. Brain imaging studies have primarily been focused on adults. In general, during stuttering, cerebral activities change dramatically in comparison to silent rest or fluent speech between people who stutter and people who do not stutter.
Studies utilizing positron emission tomography (PET) have found during tasks that invoke disfluent speech, people who stutter show hypoactivity in cortical areas associated with language processing, such as Broca's area, but hyperactivity in areas associated with motor function.[15] One such study that evaluated the stutter period found that there was over activation in the cerebrum and cerebellum, and relative deactivation of the left hemisphere auditory areas and frontal temporal regions.[40]
In non-stuttering, normal speech, PET scans show that both hemispheres are active but that the left hemisphere may be more active. By contrast, people who stutter yield more activity on the right hemisphere, suggesting that it might be interfering with left-hemisphere speech production. Another comparison of scans anterior forebrain regions are disproportionately active in stuttering subjects, while post-rolandic regions are relatively inactive.[41]
Functional magnetic resonance imaging (fMRI) has found abnormal activation in the right frontal operculum (RFO), which is an area associated with time-estimation tasks, occasionally incorporated in complex speech.[15]
Researchers have explored temporal cortical activations by utilizing magnetoencephalography (MEG). In single-word-recognition tasks, people who do not stutter showed cortical activation first in occipital areas, then in left inferior-frontal regions such as Broca’s area, and finally, in motor and premotor cortices. The people who stutter also first had cortical activation in the occipital areas, but, interestingly, the left inferior-frontal regions were activated only after the motor and premotor cortices were activated.[15][40]
It is important to note that the neurological abnormalities found in adults does not conclude if childhood stuttering caused these abnormalities or if the abnormalities cause stuttering.[34] Future research should address a longitudinal case study to track the development of brain structure in relation to stuttering.
Physiopathology of developmental stuttering [edit]

Much evidence from neuroimaging techniques has supported the theory that the right-hemisphere of people who stutter interferes with left-hemisphere speech production. Additionally, people who stutter seem to activate motor programs before the articulatory or linguistic processing is initiated.
Overactivity and underactivity [edit]
During speech production, people who stutter show overactivity in the anterior insula, cerebellum and bilateral midbrain. They show underactivity in the ventral premotor, Rolandic opercular and sensorimotor cortex bilaterally and Heschl’s gyrus in the left hemisphere.[42] Additionally, speech production in people who stutter yields underactivity in cortical motor and premotor areas.[34]
Anatomical differences [edit]
Though neuroimaging studies have not yet found specific cortical correlates, there is much evidence that there are differences in the brain physiology of adults who stutter in comparison to those who do not.
Asymmetry has been found between the left and right planum temporale in comparing people who stutter and people who do not stutter.[40] These studies have also found that there are anatomical differences in the Rolandic operculum and arcuate fasciculus.[citation needed]
Dopamine abnormalities [edit]
Recent studies have found that adults who stutter have elevated levels of the neurotransmitter dopamine, and have thus found dopamine antagonists that reduce stuttering (see anti-stuttering medication below).[40] Overactivity of the midbrain has been found at the level of the substantia nigra extended to the red nucleus and subthalamic nucleus, which all contribute to the production of dopamine.[34]
Treatment [edit]

Fluency shaping therapy [edit]
Fluency shaping therapy, also known as "speak more fluently", "prolonged speech", or "connected speech", trains people who stutter to speak fluently by controlling their breathing, phonation, and articulation (lips, jaw, and tongue). It is based on operant conditioning techniques.[43]
People who stutter are trained to reduce their speaking rate by stretching vowels and consonants, and using other fluency techniques such as continuous airflow and soft speech contacts. The result is very slow, monotonic, but fluent speech, used only in the speech clinic. After the person who stutters masters these fluency skills, the speaking rate and intonation are increased gradually. This more normal-sounding, fluent speech is then transferred to daily life outside the speech clinic, though lack of speech naturalness at the end of treatment remains a frequent criticism. Fluency shaping approaches are often taught in intensive group therapy programs, which may take two to three weeks to complete, but more recently the Camperdown program, using a much shorter schedule, has been shown to be effective.[44]
Stuttering modification therapy [edit]
The goal of stuttering modification therapy is not to eliminate stuttering but to modify it so that stuttering is easier and less effortful.[45] The rationale is that since fear and anxiety causes increased stuttering, using easier stuttering and with less fear and avoidance, stuttering will decrease. The most widely known approach was published by Charles Van Riper in 1973 and is also known as block modification therapy.[46] However, depending on the patient, speech therapy may be ineffective.[47]
Electronic fluency device [edit]

Altered auditory feedback, so that people who stutter hear their voice differently, has been used for over 50 years in the treatment of stuttering.[48] Altered auditory feedback effect can be produced by speaking in chorus with another person, by blocking out the person who stutter's voice while talking (masking), by delaying the person who stutter's voice slightly (delayed auditory feedback) and/or by altering the frequency of the feedback (frequency altered feedback). Studies of these techniques have had mixed results, with some people who stutter showing substantial reductions in stuttering, while others improved only slightly or not at all.[48] In a 2006 review of the efficacy of stuttering treatments, none of the studies on altered auditory feedback met the criteria for experimental quality, such as the presence of control groups.[49]
Anti-stuttering medications [edit]
The effectiveness of pharmacological agents, such as benzodiazepines, anticonvulsants, antidepressants, antipsychotic and antihypertensive medications, and dopamine antagonists in the treatment of stuttering has been evaluated in studies involving both adults and children.[50] A comprehensive review of pharmacological treatments of stuttering in 2006 concluded that few of the drug trials were methodologically sound.[50] Of those that were, only one, not unflawed study,[51] showed a reduction in the frequency of stuttering to less than 5% of words spoken. In addition, potentially serious side effects of pharmacological treatments were noted,[50] such as weight gain and the potential for blood pressure increases. There is one new drug studied especially for stuttering named pagoclone, which was found to be well-tolerated "with only minor side-effects of headache and fatigue reported in a minority of those treated".[52]
Support groups and the self-help movement [edit]
With existing behavioral, prosthetic, and pharmaceutical treatments providing limited relief from the overt symptoms of stuttering, support groups and the self-help movement continue to gain popularity and support by professionals and people who stutter. One of the basic tenets behind the self-help movement is that since a cure does not exist, quality of life can be improved by not thinking about the stammer for prolonged periods. Psychoanalysis has claimed success in the treatment of stuttering.[53] Hypnotherapy has also been explored as a management alternative.[54][55][56] Support groups further focus on the fact that stuttering is not a physical impediment but a psychological one.[57]
Diaphragmatic breathing [edit]
Several treatment initiatives advocate diaphragmatic breathing (or costal breathing) as a means by which stuttering can be controlled. Performing vocal artists[clarification needed], who have strengthened their diaphragm, tend to stutter when speaking but not when singing because singing involves voluntary diaphragm usage while speaking involves involuntary diaphragm usage primarily.[58][59]
Prognosis [edit]

Among preschoolers, the prognosis for recovery is good. Based on research, about 65% of preschoolers who stutter recover spontaneously in the first two years of stuttering,[18][60] and about 74% recover by their early teens.[61] In particular, girls seem to recover well.[61][62] For others, early intervention is effective in helping the child achieve normal fluency.[63]
Once stuttering has become established, and the child has developed secondary behaviors, the prognosis is more guarded,[63] and only 18% of children who stutter after five years recover spontaneously.[64] However, with treatment young children may be left with little evidence of stuttering.[63]
With adult people who stutter, there is no known cure,[61] though they may make partial recovery or even complete recovery with intervention. People who stutter often learn to stutter less severely and be less affected emotionally, though others may make no progress with therapy.[63]
Epidemiology [edit]

The lifetime prevalence, or the proportion of individuals expected to stutter at one time in their lives, is about 5%,[65] and overall males are affected two to five times more often than females.[17][66][67] Most stuttering begins in early childhood, and studies suggest that 2.5% of children under the age of 5 stutter.[68][69] The sex ratio appears to widen as children grow: among preschoolers, boys who stutter outnumber girls who stutter about two to one, or less.[67][69] but widens to three to one at first grade and five to one at fifth grade,[70] due to higher recovery rates in girls.[61] Due to high (approximately 65–75%) rates of early recovery,[66][71] the overall prevalence of stuttering is generally considered to be approximately 1%.[17][72]
Cross-cultural studies of the stuttering prevalence were very active in early and middle of the 20th century, particularly under the influence of the works of Wendell Johnson, who claimed that the onset of stuttering was connected to the cultural expectations and the pressure put on young children by anxious parents. Johnson claimed there were cultures where stuttering, and even the word "stutterer", were absent (for example, among some tribes of American Indians). Later studies found that this claim was not supported by the facts, so the influence of cultural factors in stuttering research declined. It is generally accepted by contemporary scholars that stuttering is present in every culture and in every race, although the attitude towards the actual prevalence differs. Some believe stuttering occurs in all cultures and races[26] at similar rates,[17] about 1% of general population (and is about 5% among young children) all around the world. A US-based study indicated that there were no racial or ethnic differences in the incidence of stuttering in preschool children.[68][69] At the same time, there are cross-cultural studies indicating that the difference between cultures may exist. For example, summarizing prevalence studies, E. Cooper and C. Cooper conclude: "On the basis of the data currently available, it appears the prevalence of fluency disorders varies among the cultures of the world, with some indications that the prevalence of fluency disorders labeled as stuttering is higher among black populations than white or Asian populations" (Cooper & Cooper, 1993:197).
Different regions of the world are researched very unevenly. The largest number of studies had been conducted in European countries and in North America, where the experts agree on the mean estimate to be about 1% of the general population (Bloodtein, 1995. A Handbook on Stuttering). African populations, particularly from West Africa, might have the highest stuttering prevalence in the world—reaching in some populations 5%, 6% and even over 9%.[73] Many regions of the world are not researched sufficiently, and for some major regions there are no prevalence studies at all (for example, in China). Some claim the reason for this might be a lower incidence in general population in China.[74]


Lewis Carroll, the well-known author of Alice's Adventures in Wonderland, was afflicted with a stammer, as were his siblings.

Because of the unusual-sounding speech that is produced and the behaviors and attitudes that accompany a stutter, it has long been a subject of scientific interest and speculation as well as discrimination and ridicule. People who stutter can be traced back centuries to the likes of Demosthenes, who tried to control his disfluency by speaking with pebbles in his mouth.[75] The Talmud interprets Bible passages to indicate Moses was also a person who stuttered, and that placing a burning coal in his mouth had caused him to be "slow and hesitant of speech" (Exodus 4, v.10)[75]
Galen's humoral theories were influential in Europe in the Middle Ages for centuries afterward. In this theory, stuttering was attributed to imbalances of the four bodily humors—yellow bile, blood, black bile, and phlegm. Hieronymus Mercurialis, writing in the sixteenth century, proposed methods to redress the imbalance including changes in diet, reduced lovemaking (in men only), and purging. Believing that fear aggravated stuttering, he suggested techniques to overcome this. Humoral manipulation continued to be a dominant treatment for stuttering until the eighteenth century.[76] Partly due to a perceived lack of intelligence because of his stutter, the man who became the Roman Emperor Claudius was initially shunned from the public eye and excluded from public office.[75]
In and around eighteenth and nineteenth century Europe, surgical interventions for stuttering were recommended, including cutting the tongue with scissors, removing a triangular wedge from the posterior tongue, and cutting nerves, or neck and lip muscles. Others recommended shortening the uvula or removing the tonsils. All were abandoned due to the high danger of bleeding to death and their failure to stop stuttering. Less drastically, Jean Marc Gaspard Itard placed a small forked golden plate under the tongue in order to support "weak" muscles.[75]

Italian pathologist Giovanni Morgagni attributed stuttering to deviations in the hyoid bone, a conclusion he came to via autopsy.[76] Blessed Notker of St. Gall (ca. 840–912), called Balbulus ("The Stutterer") and described by his biographer as being "delicate of body but not of mind, stuttering of tongue but not of intellect, pushing boldly forward in things Divine," was invoked against stammering.
Famous Englishmen who stammered were King George VI and Prime Minister Winston Churchill, who led the UK through World War II.
George VI went through years of speech therapy, most successfully under Australian speech therapist Lionel Logue, for his stammer. This is dealt with in the Academy Award-winning film The King's Speech (2010) in which Colin Firth plays George VI. The film is based on an original screenplay by David Seidler who also used to stutter as a child until age 16.[77]
Churchill claimed, perhaps not directly discussing himself, that "[s]ometimes a slight and not unpleasing stammer or impediment has been of some assistance in securing the attention of the audience..."[78] However, those who knew Churchill and commented on his stutter believed that it was or had been a significant problem for him. His secretary Phyllis Moir commented that "Winston Churchill was born and grew up with a stutter" in her 1941 book I was Winston Churchill's Private Secretary. She also noted that about one incident 'It’s s s simply s s splendid" he stuttered, as he always did when excited.’ Louis J. Alber, who helped to arrange a lecture tour of the United States wrote in Volume 55 of The American Mercury (1942) that "Churchill struggled to express his feelings but his stutter caught him in the throat and his face turned purple" and that "born with a stutter and a lisp, both caused in large measure by a defect in his palate, Churchill was at first seriously hampered in his public speaking. It is characteristic of the man’s perseverance that, despite his staggering handicap, he made himself one of the greatest orators of our time."
For centuries "cures" such as consistently drinking water from a snail shell for the rest of one's life, "hitting a stutterer in the face when the weather is cloudy", strengthening the tongue as a muscle, and various herbal remedies were used.[79] Similarly, in the past people have subscribed to theories about the causes of stuttering which today are considered odd. Proposed causes of stuttering have included tickling an infant too much, eating improperly during breastfeeding, allowing an infant to look in the mirror, cutting a child's hair before the child spoke his or her first words, having too small a tongue, or the "work of the devil."[79]

Jazz and Eurodance musician Scatman John wrote the song "Scatman (Ski Ba Bop Ba Dop Bop)" to help children who stutter overcome adversity. Born John Paul Larkin, Scatman spoke with a stutter himself and won the American Speech-Language-Hearing Association's Annie Glenn Award for outstanding service to the stuttering community.[80]
Fiction character Albert Arkwright from British sitcom Open All Hours, stammered and much of the series' humour revolved around this. Recurring character Reginald Barclay from the Star Trek television franchise and the Emperor Claudius from the I, Claudius series by Robert Graves and acted by Derek Jacobi are portrayed as suffering from and overcoming their stuttering.
Cartoon character Porky Pig has a notable stutter. This arose because his original voice artist, Joe Dougherty, had an authentic stammer. However, Dougherty's stutter caused recording sessions to take longer than otherwise necessary, and so Warner Bros. replaced him with Mel Blanc, who provided Porky's voice for the rest of his life. Porky's stutter is probably most pronounced when he says "Th-th-th-that's all, folks!" Also a person who stutters is the cartoon character Keswick from T.U.F.F. Puppy.

A lisp, also known as sigmatism, is a speech impediment whose sufferers are unable to articulate sibilants ([s], [z], [ʒ], [ʃ], [tʃ], [dʒ]).[1] These misarticulations often result in unclear speech.

Type of lisps [edit]
"Interdental" lisping is produced when the tip of the tongue protrudes between the front teeth and "dentalised" lisping is produced when the tip of the tongue just touches the front teeth.
The "lateral" lisp is where the [s] and [z] sounds are produced as a result of air-flow being pushed over the sides of the tongue. It is also called 'slushy ess' or a 'slushy lisp' due to the wet, spitty sound. This is fine when it comes to producing the "L" tones, but it sounds slushy or wet when trying to make out the "S" tone. The symbols for these lateralised sounds are in the Extended International Phonetic Alphabet for disordered speech ([ʪ] and [ʫ]).
A "nasal lisp" occurs when part or the entire air stream is directed through the nasal cavity.
A "trident lisp" results in a high-frequency whistle of hissing sound caused by stream passing between the tongue and the hard surface.
A "dentalized lisp" does not have a sibilant quality, resulting from occluding the breath stream in the oral cavity, usually by placing the tongue too far forward of not allowing for a narrow, groved passageway for the breath stream to pass between the tongue and alveolar ridge.
A "palatal lisp", is where the speaker attempts to make the sounds with the middle of the tongue coming into contact with the soft palate.[1]
Cause of lisps edit

Lisps may have either physical or psychological causes. Most lisps are caused by errors in tongue placement within the mouth. The most frequently discussed of these problems is tongue thrust in which the tongue protrudes beyond the anterior incisors. This protrusion affects speech as well as swallowing and can lead to lisping. Ankyloglossia or tongue-tie can also be responsible for lisps in children. However it is unclear whether these deficiencies are caused by the tongue-tie itself or the muscle weakness following the correction of the tongue-tie.[2] Overbites and underbites may also contribute to lisping. Temporary lisps can be caused by dental work, dental appliances such as dentures or retainers or by swollen or bruised tongues.
Treatment Treating lisps in children usually involves speech therapy treatments and is generally successful. Speech therapy sessions include a wide variety of activities and speech drills, though what specifically happens in any given session will depend upon many variables. The content of these sessions will be affected by the length of each therapy session (usually between a half hour and one hour), its location (whether at home, school or a private facility), the age of the child involved, whether the therapy session is private or involves a group, and the type of lisp that is being treated.
One popular method of correcting articulation or lisp disorders is to isolate sounds and work on correcting the sound in isolation. The basic sound, or phoneme, is selected as a target for treatment. Typically the position of the sound within a word is considered and targeted. The sound appears in the beginning of the word, middle, or end of the word (initial, medial, or final).
Take for example, correction of an "S" sound (lisp). Most likely, a speech-language pathologist (SLP) would employ exercises to work on "Sssssss". Starting practice words would most likely consist of "S-initial" words such as "say, sun, soap, sip, sick, said, sail". According to this protocol, the SLP slowly increases the complexity of tasks (context of pronunciations) as the production of the sound improves. Examples of increased complexity could include saying words in phrases and sentences, saying longer multi-syllabic words, or increasing the tempo of pronunciation.
Using this method, the SLP achieves success with his/her student by targeting a sound in a phonetically consistent manner. Phonetic consistency means that a target sound is isolated at the smallest possible level (phoneme, phone, or allophone) and that the context of production must be consistent. Consistency is critical, because factors such as the position within the word, grouping with other sounds (vowels or consonants), and the complexity all may affect production.
Another popular method for treating a lisp is using specially designed devices that go in the mouth to provide a tactile cue of exactly where the tongue should be positioned when saying the "S" sound. This tactile feedback has been shown to correct lisp errors twice as fast as traditional therapy.
Using either or both methods, the repetition of consistent contexts allows the student to align all the necessary processes required to properly produce language; language skills (ability to formulate correct sounds in the brain: What sounds do I need to make?), motor planning (voicing and jaw and tongue movements: How do I produce the sound?), and auditory processing (receptive feedback: Was the sound produced correctly? Do I need to correct?). A student with an articulation or lisp disorder has a deficiency in one or more of these areas. To correct the deficiency, adjustments have to be made in one or more of these processes. The process to correct it is more often than not, trial and error. With so many factors, however, isolating the variables (the sound) is imperative to getting to the end result faster.
A phonetically consistent treatment strategy means practicing the same thing over and over. What is practiced is consistent and does not change. The words might change, but the phoneme and its positioning is the same (say, sip, sill, soap, …). Thus, successful correction of the disorder is found in manipulating or changing the other factors involved with speech production (tongue positioning, cerebral processing, etc.). Once a successful result (speech) is achieved, then consistent practice becomes essential to reinforcing correct productions.
When the difficult sound is mastered, the child will then learn to say the sound in syllables, then words, then phrases and then sentences. When a child is able to speak a whole sentence without lisping, attention is then focused on making correct sounds throughout natural conversation. Towards the end of the course of therapy, the child will be taught how to monitor his or her own speech, and how to correct as necessary. Speech therapy can sometimes fix the problem, but however in some cases speech therapy fails to work.
Permanent lisps can often be corrected through extensive oral operations. Often, when a patient has extreme overbite, causing a lisp, having orthodontic braces and rubber bands for an extended period of time will correct the issue, and resolve the lisp.

There is a long tradition of interest in communication disorder. From
Demosthenes with his pebbles, from Leonardo da Vinci with his drawings
and models of the vocal tract, from Pedro Ponce de Leon with his early
version of ' Total communication' in sixteenth-century Spain, and from the
divergence of opinion of L'Abbe de L'Eppee and Heineke in the
eighteenth century, students of speech pathology know that the inability of
some to use communication normally has always fascinated. However, this
historical review will be restricted to a very brief outline of changes in
emphases of assessment over the twentieth century.
In the early part of this century, speech and language therapists fell
broadly into two groups. The first was concerned with the mechanics of
speech and embraced the new knowledge derived from phonetics (e.g.
Aiken, 1910). Aiken's preface to the second edition of his book The Voice
states that ' it has become necessary to remodel and rewrite the whole work
and to regard it as an introduction to the practical science of phonology'
(p. I). The second group concentrated more on the psychological behaviour
of 'speech-disordered' people. Researchers in speech pathology,
notably in Berlin following Gutzman, in Vienna following Froeschels, in
London and in the United States, notably at the University of Columbia,
following Scripture, were fascinated by four problems: those of stuttering,
of voice, of 'dyslalia' and of dysphasia, almost to the exclusion of any
awareness of other disorders. Similarly the remedial practitioners, who in
the early part of the century came from a range of backgrounds, became
remedial specialists for specific client groups.

Had  a girl with  stammering problem come to USA from Hyderabad India  to undergo  speech and language  pathology treatment. one  way this  is being  corrected is  by  a device  placed in  her  one  ear which delays and also changes the tone and frequency of her own speech as feed back somehow this  correct the  stammering.
I do not have  any objective test results available but   My brother tells me  it  is working  quite well.
this  is  being  done by a  speech therapist in College station Texas.

Poor adherence to appropriate medication therapy has been shown to result in complications, death, and increased health care costs.1-4 Medication adherence in patients with chronic conditions such as diabetes, hypertension, hyperlipidemia, asthma, and depression is a significant problem requiring intervention.5-12 According to the World Health Organization’s (WHO) World Health Report 2003, the degree of medication nonadherence is so great and the consequences are of such concern that more people worldwide would benefit from efforts to improve medication adherence than from the development of new medical treatments.13 In fact, a number of studies have shown that, in developed countries, patients with chronic conditions have adherence rates of 50% to 60%, despite evidence that medication improves quality of life and prevents death.5-10 In developing countries, when poor access to health care, possible lack of diagnosis, and limited availability of medications are taken into account, poor adherence threatens all efforts to treat chronic conditions such as diabetes, depression, and HIV/AIDS.11
The rate of patients who persistently take their medication for a chronic condition drops most significantly after the first 6 months of therapy.14-17 Acceptable rates of adherence in some trials are 80% or greater, while those for HIV are considerably higher (95%).17 Of all medication-related hospital admissions in the United States, 33% to 69% are related to poor medication adherence. The resulting costs are approximately $100 billion annually.17 It has been estimated that 125,000 deaths per year in the U.S. can be attributed to medication nonadherence.18 A retrospective analysis of patients aged under 65 years that was conducted between 1997 and 1999 concluded that diabetic patients with poor medication adherence had a 30% yearly risk of hospitalization, while those with high adherence (defined as following the medication regimen 80%-100% of the time) had a 13% risk.19 In the same analysis, total health care spending for patients with low medication adherence was almost twice that of patients with high adherence.
It was estimated in 2001 that more than 89,000 deaths from cardiovascular disease would have occurred in hypertensive patients aged 40 years and older who were not receiving antihypertensive medication therapy.20 Patients who are diabetic or who have heart disease are 48% less likely to die prematurely when they are adherent to their medication regimen.21 In addition, asthma patients who are adherent to their medications are 11% less likely to have an emergency room (ER) visit or to be hospitalized.21
When counseling about medication adherence for the three major disease states—diabetes, hypertension, and hyperlipidemia—it is important to educate the patient about the consequences of not appropriately treating these conditions, which can result in heart attack, heart failure, stroke, paralysis, and potentially permanent and devastating alterations to their quality of life. Pharmacists must paint a picture of the situation and remind patients why they are taking the medication in the first place.

Defining Adherence

It has been said that a primary determinant of treatment success is adherence to therapy.22 As defined by the WHO, adherence is the degree to which the patient’s behavior is in agreement with the health care provider’s recommendations. This is a collaborative approach between the patient and the provider in which the patient’s values, lifestyle, and beliefs coincide with the provider’s medical advice and opinion. Compliance, which is often confused with adherence, refers to a patient’s obedience to therapy under the authority of the provider, rather than to a collaborative approach. Another term that relates to adherence is persistence, which is the patient’s ability to continue taking the medication for the duration of treatment. When a medication is initiated, discussing the intended course of therapy with the patient has been shown to help keep the patient persistently following his or her medication regimen.12

Factors That Impact Adherence

The most consistently reported factors impacting adherence are low literacy, lack of health insurance coverage, poor social support, family instability, and homelessness.12 Forty-five percent of U.S. adults have a literacy level at or below eighth grade.12 It has been reported that one-half of adults lack the skills to complete tasks related to following medication-label directions, comprehending nutrition labels, describing symptoms, or using a map to locate health facilities.23 This can be a particular problem in ethnic groups that already have a low English-literacy level. Decreased health literacy has been reported with increasing age, such that adults aged 60 years may not understand basic materials such as medication labels. See TABLE 1 for tools used both to assess health literacy, medication literacy, and willingness to change behaviors and to evaluate the degree of social support available to the patient.24-27 Patient beliefs about the diagnosis and medication benefits also have been shown to affect the adherence rate regardless of whether the patient can afford the medication. This means that patients may have a low perceived need for the medication and more concerns about side effects, thus possibly choosing to forgo the cost of the medication based on their beliefs.28

As providers, pharmacists can use the patient education program Ask Me 3, which is designed to promote communication between health care providers and patients, to improve adherence and outcomes. The Ask Me 3 questions are: What is my main problem? What do I need to do? and Why is it important for me to do this?29 Pharmacists should instruct their patients to ask these three questions, and they should state them to the patient and answer them in the event that the patient does not ask. It is equally important that the patient rephrase what the pharmacist has communicated in order to verify the patient’s comprehension and to clarify whether the information fits the patient’s beliefs. When answering these questions, particularly with asymptomatic patients, it is helpful for pharmacists to remind patients of the consequences of failing to take their medication, such as a devastating or life-ending event (heart attack or stroke) in patients with cardiovascular disease.
Prerequisites for medication adherence may include being interested in one’s health and understanding the diagnosis; comprehending the impact or potential impact of the diagnosis; believing that what has been prescribed will help; and knowing precisely how to take the medication and for how long. Other prerequisites may be determining how to fit the medication into one’s daily routine; valuing the results of treatment more than the cost of therapy; believing that one can fulfill the treatment plan; and feeling that the provider has true concern for the patient as a person, as opposed to just treating the disease.12
The following steps have been shown to increase adherence: verifying the patient’s understanding of the disease and its treatment and providing education where gaps in understanding exist; correlating medication-taking with other daily routines; using medication organizers or charts; reducing the pill burden, if possible; providing care support (i.e., health care team); and recognizing any struggles or other sociobehavioral issues that may affect adherence.12

How to Overcome Adherence Challenges

Many different factors can affect adherence. The first step, and the one that is most often missed, is to identify which factors are preventing adherence. One way to identify adherence gaps is to perform a medication reconciliation—best done using pharmacy adjudication records—allowing for discovery of pharmacy-dispensing gaps. This might reveal the need for a simplification of the dosing regimen.30 See TABLE 2 for a list of factors that affect adherence and strategies for overcoming them.29,31,32

Strategies to overcome low health literacy include creating a shame-free environment that promotes open dialogue; using plain language versus medical terminology; providing written communication on a fifth-grade level; using picture or video instruction; involving family members in the instruction; having the patient repeat the instruction; and following up by telephone to confirm the patient’s understanding.12
The inability to pay for health care can be a deterrent to adequate adherence; however, the degree to which patients value their medication or perceive it to help their condition, along with their beliefs about side effects, can influence their decision to forgo treatment, regardless of cost.28 Patients with depression show a greater tendency to be deterred by their beliefs and medication costs, thereby putting those with diabetes and depression at even greater risk for medication nonadherence. This is true for patients who have depression with other comorbid conditions. Providers should initiate more in-depth discussions about medication cost concerns in patients with depression.28
Lack of prescription coverage can be a major roadblock to medication adherence. There are several options for patients who do not have prescription coverage, including Medicare Part D (for patients eligible for Medicare), Medicare Extra Help (for those who have Medicare but have limited income and resources), Medicaid programs, the Together Rx Access Card (25%-40% off brand-name prescription products), Partnership for Prescription Assistance (free medications to qualified patients), NeedyMeds Drug Discount Card (80% off the price of prescriptions), Coast2Coast Rx Card (free diabetes supplies and up to 55% off medication costs), and discount cards offered by some pharmacy benefit management companies and individual pharmaceutical companies.33-40 This list of discount cards is not all-inclusive. The $4 generic program is another option offered by some large chain pharmacies and supermarket pharmacies; in some cases, independent pharmacies offering this program may match pricing.41 All of these discount programs have restrictions, so check their respective Web sites (TABLE 3).33-41 Lastly, some employers offer reduced copayments or medication deductibles to patients who participate in a health program for chronic diseases.42

Conclusion

Poor adherence to medication regimens is multifactorial, involving the patient’s beliefs about and understanding of the diagnosis and how the medication will help his or her prognosis.22 Three important questions to answer for the patient are: What is the patient’s main problem? What does the patient need to do? and Why is it important for the patient to do this? After these questions have been answered, ascertain that the patient believes that this treatment will help. Afterward, probe for any issues that may result in low adherence, such as low literacy, inability to pay for the medication, and poor social support, among many others.29 The first step is to check medication adherence and persistence by way of a pharmacy medication reconciliation that utilizes pharmacy adjudication claims.12

Abstract

BACKGROUND:

People who are prescribed self-administered medications typically take less than half the prescribed doses. Efforts to assist patients with adherence to medications might improve the benefits and efficiency of health care, but also might increase its adverse effects.

OBJECTIVES:

To update a review summarising the results of randomised controlled trials (RCTs) of interventions to help patients follow prescriptions for medications for medical problems, focusing on trials that measured both adherence and clinical outcomes.

SEARCH STRATEGY:

Computerised searches to August 2001 in MEDLINE, CINAHL, The Cochrane Library, International Pharmaceutical Abstracts (IPA) PsychInfo, and Sociofile; bibliographies in articles on patient adherence; articles in the reviewers' personal collections; and contact with authors of original and review articles on the topic.

SELECTION CRITERIA:

Articles were selected if they reported an unconfounded RCT of an intervention to improve adherence with prescribed medications, measuring both medication adherence and treatment outcome, with at least 80% follow-up of each group studied and, for long-term treatments, at least six months follow-up for studies with positive initial findings.

DATA COLLECTION AND ANALYSIS:

Information on study design features, interventions and controls, and results were extracted by one reviewer and confirmed by at least one other reviewer. The studies were too disparate to warrant meta-analysis.

MAIN RESULTS:

For short-term treatments, one of three interventions reported in three RCTs showed an effect on both adherence and clinical outcome. Eighteen of 36 interventions for long-term treatments reported in 30 RCTs were associated with improvements in adherence, but only 16 interventions led to improvements in treatment outcomes. Almost all of the interventions that were effective for long-term care were complex, including combinations of more convenient care, information, reminders, self-monitoring, reinforcement, counselling, family therapy, and other forms of additional supervision or attention by a health care provider (physician, nurse, pharmacist or other). Even the most effective interventions did not lead to large improvements in adherence and treatment outcomes. Two studies showed that telling patients about adverse effects of treatment did not affect their adherence.

REVIEWER'S CONCLUSIONS:

The full benefits of medications cannot be realised at currently achievable levels of adherence. Current methods of improving adherence for chronic health problems are mostly complex and not very effective. Innovations to assist patients to follow medication prescriptions are needed.
